1) Demolisher dies in the beginning battle with Sideways. Well to answer your question Optimus and Ironhide in a way killed him. Before he dies he says "This is not your planet to rule.......The Fallen-the Fallen...shall rise again."

2) Um no well besides Soundwave and some small robots that sam encounters in his kitchen when a small fragmint of the AllSpark sparks in the kitchen and those robots are born.

SPOILER ALERT:
The final battle takes place in Egypt where Devastator is killed by Seymor Simmmons by sending F-22 I believe and they kill him. Jetfire sacrifices himself and combines with Optimus prime (yes the combine) and so Optimus is now stonger and defeats The Fallen (though I believe he is not dead, tell you later why) and beats up Megatron and Megatron flees along with Starscream.

He dies at opening battle scene at Shagnai,China. Sideswipe kills him or was it Optimus? Pretty sure it was Sideswipe.

I already seen it at an early showing cause one of my friends work for sony so he gave me and my friends early screening but I'm still going to watch it on IMAX.
